• Open to all applicants of age 18years and above. 
• New applicants must present either of the following proof of identity: Identity Card, Driving License or Passport upon submission of application. 
• A fee of RM20 is required upon application.
• Smart Club Card is valid for 1 year and to be renewed for at a fee of RM20.
• Eligible applicants will immediately be granted membership and issue a card. 
• All personal information and document provided by the applications will be kept confidential. By submitting this form the applications agree to allow GINMIT authority to process their personal information for the purpose of applying for Smart Club Card. it is also agreed that GINMIT is also authorized by the applications to store their information in their customer database and GINMIT is liberty to contact the applicant for future promotions. 
• By submitting the application form, applicants hereby undertake to keep GINMIT informed and updated on any changes in their particulars and do hereby consent to GINMIT collect their personal information. 
• GINMIT reserves the rights to reject any applicant at its sole discretion. 

Terms and Conditions: 

• Smart Club Card is valid at all branches in Malaysia.
• Smart Club Card is not a credit card or charge card.
• Smart Club Card must be presented upon every purchase for validation to enjoy all Smart Club Membership benefits.
• GINMIT reserves the right to withdraw, suspend, cancel or terminate the Smart Club program at any time and to modify, vary or change of any of the benefits from time to time without prior notice to members, even though the changes might affect the members right. 
• Should your information need to be updated due to changes, kindly visit the nearest GINMIT branch's customer service counter. This is to ensure you still enjoy the benefits as a member. 
• GINMIT reserves the rights to change, add, amend or delete any of the TERMS & CONDITIONS without prior notice and the member shall be bound by such changes.
